We would like to welcome you and your child to Hertford County Schools! Please read through the information below to identify the documents required for enrollment. Complete all forms within this online packet in order to ensure a prompt enrollment for your child. ENROLLMENT WILL NOT BE COMPLETED UNTIL ALL DOCUMENTATION IS UPLOADED VIA THE FAMILY DASHBOARD.
Hertford County Schools Pre-K Enrollment Information:
The following are required documents for all NC Pre-K Students:
- Copy of Child's Birth Certificate (Must be 4 years old on or before August 31st)
- Up-to-Date Immunization Records
- Parents are required to provide current immunization shot records. NC Child Care Laws require that immunization shot records are up-to-date. All parents are asked to update their child's file as needed.
- Children must have these shots to attend Pre-K: 4 DTaP, 3 Hep B, 3-4 Hib, 1 MMR, 3 Polio, 4 PCV, 1 Var
- Children's Medical Report
- Must be completed and signed by licensed physician AND within the last 12 months

- Current Proof of Income
- Copy of 1040 or W2 from 2021 or Pay Stubs (4 if paid weekly, 2 if paid Bi-Weekly/Semi-Monthly, 1 if paid Monthly)
- Proof of Current Residence
- Light Bill, Rental/Lease Agreement, Mortgage Statement
- Other
- Affidavit of Parent/Legal Guardian
- This form MUST be notarized

- IEP, letter from doctor or therapist currently providing services to child, developmental screenings
- If applicable:
- Copy of Legal Guardianship papers
- Statement of No Income Form
